I use the 'approve comments' hack.

Nothing gets through untill I have approved it.
I also use the Block ip and Ban user  in 'Comments Manager Module'

I find the approve comments hack to be good, although it can be a bit of a burden on large sites with lots of new commenters. For my site I use a custom anti-spam question to help prevent this.
